Am I still a winner?

I got a boxed Astro on Sunday, when I looked inside it, it had with it an orange entry form/questionnaire the prize was a Flock Astro, I want to know could I still be a winner.

I know really that I'm not but I thought I'd share it with the group.

I have never seen one of these bafore and I think they must be very rare as most people would have sent theirs in to try and win.
