This is  basically the "Psyche-De-Lite" sold exclusively to the Blues Magoos fan club, but in black wrinklepaint. Yes, it's also the Expo '67 lamp. Beyond that, nothing is known yet. A  few of these and one Psyche-De-Lite are known to exist.

Not sure if this is possible, it looks like the one pictured on the Blue Magoos single from 1967. And later their Psyche-De-Lite lamp shown on lower left guy is holding (black base lamp):

Maybe it was from an initial batch and some ended up with Expo67 logos added? Maybe never marketed as an official model.
